Breathwork Without Breath: New Study Reveals How Intentional Breathing Unlocks Psychedelic States in the Brain
In what researchers are calling a "paradigm-shifting" discovery, a new study has provided the first neuroimaging evidence that high ventilation breathwork (HVB) — a form of fast, cyclical breathing practiced for millennia in yogic and shamanic traditions — can reliably induce altered states of consciousness indistinguishable from those produced by psychedelic substances like psilocybin.
Published in the open-access journal PLOS One on August 27, 2025, the study led by Amy Amla Kartar of the Colasanti Lab at Brighton and Sussex Medical School marks the first time researchers have used functional MRI to map what happens inside the brain during breathwork-induced altered states.
What the Science Shows
The research team analyzed data from 42 experienced breathwork practitioners across three settings — online, in-lab, and inside an MRI scanner. Participants engaged in 20- to 30-minute sessions of cyclic breathing without pauses, accompanied by music, followed by detailed questionnaires within 30 minutes of completing their session.
The findings were striking. High ventilation breathwork produced what scientists call "Oceanic Boundlessness" (OBN) — a term originally coined by Freud in 1920 that describes a constellation of feelings including spiritual experience, insightfulness, bliss, positively experienced depersonalization, and a profound sense of unity. OBN is widely considered the defining feature of altered states induced by psychedelic substances.
"Our research is the first to use neuroimaging to map the neurophysiological changes that occur during breathwork," the authors stated. "We found that more profound changes in blood flow in specific brain areas were linked to deeper sensations of unity, bliss, and emotional release."
Using MRI scans, the team observed that HVB triggered a profound decrease in blood flow to the left operculum and posterior insula — brain regions responsible for representing the internal state of the body, including breathing. Simultaneously, blood flow progressively increased to the right amygdala and anterior hippocampus, areas central to emotional memory processing.
These neurological changes correlated directly with the intensity of the psychedelic experience reported by participants. Notably, all participants reported a reduction in fear and negative emotions following their sessions, with no adverse reactions recorded across any of the experimental settings.
Bridging Ancient Wisdom and Modern Science
The findings validate what spiritual traditions have known for thousands of years. Breathwork — known as pranayama in yogic philosophy — has long been regarded as a direct path to expanded awareness. The Hindu Upanishads, some of the world's oldest spiritual texts, describe breath as the bridge between the physical and the transcendent, while Taoist and Buddhist traditions have used controlled breathing as a core meditation technique for millennia.
"Many people anecdotally recognize the health benefits of breathwork," said lead author Amy Kartar. "This style of fast-paced breathing has received very little scientific attention until now."
The study's findings arrive at a time when interest in non-pharmacological approaches to consciousness exploration is surging. High ventilation breathwork is accessible, low-cost, and can be practiced safely in group settings or guided online sessions. While the authors caution that the study was exploratory and requires replication with larger sample sizes and control groups, the implications for mental health treatment are considerable.
"Breathwork practices that increase ventilatory rate or depth, accompanied by music, can lead to altered states of consciousness similar to those evoked by psychedelic substances," the researchers noted. "HVB might offer a non-pharmacological alternative, with fewer legal and ethical restrictions to large-scale adoption in clinical treatment."
